About The Role

We are seeking a compassionate and skilledNurse Practitioner (NP)to deliver high-quality, patient-centered primary care services to residents in assisted living and long-term care facilities. This is a meaningful opportunity for providers who are dedicated to improving the health and quality of life for older adults in a supportive, team-based environment.

Key Responsibilities

• Conduct comprehensive health assessments including physical exams, medication reconciliation, and review of medical histories.
• Develop and implement individualized care plans in collaboration with interdisciplinary teams.
• Manage chronic and acute health conditions with timely interventions and appropriate referrals.
• Order and interpret diagnostic tests (labs, imaging, etc.) to guide clinical decisions.
• Prescribe medications and treatments in accordance with state scope of practice and facility policy.
• Initiate and participate in chronic care management programs.
• Educate residents and families on health maintenance, disease prevention, and self-care.
• Participate in quality improvement efforts and interdisciplinary care meetings.
• Maintain clear, accurate documentation in the electronic medical record (EMR) system.
  
  

Qualifications

• Master’s Degree in Nursing from an accredited Nurse Practitioner program.
• Active and unrestrictedNew York State NP license.
• Valid DEA registration for prescribing.
• Eligible for reimbursement through Medicare and Medicaid.
