Overview

GQR Healthcare is seeking a Nurse Practitioner Home Health for a local contract job in Michigan, Michigan. Base pay range: $74.00/hr - $84.00/hr. Your actual pay will be based on your skills and experience — talk with your recruiter to learn more.

Start Date: October 15, 2025 (credentialing begins October 1). Duration: 13 weeks. Schedule: 40 hours per week, 8-hour shifts, days. Employment Type: Local Contract / Locum Tenens. Location: Michigan with travel across the state (focus on Kent County and surrounding regions).

Responsibilities

• Conduct solo in-home visits to health plan members (primarily seniors 65+ and select Medicaid populations 18+).
• Perform Level 2–3 Diagnostic Visits (e.g., A1C, FIT, KED, retinal exams).
• Perform Level 4 Comprehensive Annual Wellness Exams.
• Perform diagnostic screenings and collect labs.
• Assess Social Determinants of Health (SDOH).
• Collaborate with care coordination teams to deliver 30-day resource guides for members.
• Complete documentation via iPad/EHR platform (training provided).
• Ensure clinical safety and report unsafe situations to the coordination team.

Qualifications

• Active Nurse Practitioner license in Michigan (with MI RN license).
• Board Certified FNP-C or GNP (Family Practice, Internal Medicine, or Geriatrics).
• Enrolled as a Medicare and Medicaid provider in Michigan.
• Experience: Home Health experience preferred; new grads with Home Health RN background considered.
• In-home assessment and diagnostic experience preferred; familiarity with HEDIS, HRAs, risk adjustment, and quality-gap closures.
• Technology: Comfortable using iPads; Athena EHR experience a plus.
• Vehicle: Willingness to travel within assigned region (mileage/rental reimbursement available).
• Glove type: Nitrile or Latex (please specify size).
