Job summary

ConnectedHealth in Minnesota is seeking a Licensed Practical Nurse to provide overnight care in a skilled nursing facility on a consistent schedule with opportunities to pick up additional shifts.

You will administer medications, monitor residents, and collaborate with the care team to ensure high-quality, patient-centered care while maintaining documentation and following safety protocols.

Qualifications

• Active LPN license in Minnesota.
• Graduate of an accredited Practical Nursing program.
• Current BLS certification.
• Strong communication, organizational, and teamwork skills.

Qualifications:

• Previous Skilled Nursing, Long-Term Care (LTC), or Rehabilitation experience.
