Job summary

Alteas Health seeks a Nurse Practitioner or Physician Assistant to deliver specialty Cardiology services across Skilled Nursing, Long-Term Care, and Post-Acute Care facilities in the Peoria area. You'll work with physicians, facility staff, and interdisciplinary teams to provide patient-centered care.

Role offers clinical autonomy, daily travel within the territory, and collaboration with supportive leadership.

Qualifications

• Current, unrestricted NP or PA license in applicable state(s).
• Current national certification appropriate to licensure.
• Current BLS certification.
• Minimum six months of NP or PA clinical experience.
• Valid driver's license with reliable transportation.
• Ability to travel daily throughout an assigned territory.
• Strong clinical judgment, communication, organizational, and critical thinking skills.
• Experience using Electronic Medical Records (EMR).

Responsibilities

• Provide specialty Cardiology and collaboration with physicians and care teams.
• Evaluate, diagnose, and manage cardiovascular conditions.
• Develop individualized treatment plans using evidence-based practice.
• Order, interpret, and communicate labs and imaging.
• Prescribe medications and treatments within scope.
• Educate patients and caregivers on diagnoses and care plans.
• Coordinate care with physicians, nursing staff, and other professionals.
• Maintain compliant clinical documentation.
• Round at facilities within the assigned territory.
• Build relationships with facility leadership and clinical teams.
• Represent Alteas Health with professionalism and clinical excellence.
