This is a Part-Time position doing Home Visits for Hospice. Candidates must be licensed as a Social Worker in OH, with a Master's degree preferred. 20-28 hrs/week. Flexible schedule - Friday mornings required.

Responsibilities

The Medical Social Worker for hospice provides medical social services under the direction of a physician and Interdisciplinary Group to assist in understanding significant social and emotional factors related to the patient's health status and in developing coping mechanisms.

• Assists the patient, significant others, physician, and healthcare team to understand personal, emotional, environmental, and social factors related to the patient's health status as needed.
• Contributes as a healthcare team member to the development of a comprehensive, integrated Plan of Care for patients daily.
• Instructs healthcare team members on community resources available to assist patients as needed.
• Function as Bereavement Coordinator and supervise the provision of bereavement services reflecting patient and family needs if needed.
• Establishes a Plan of Care addressing bereavement needs with clear delineation of services and frequency of delivery up to thirteen (13) months following the patient's death.

Education and Experience

License Requirements

• Master's Degree from an accredited school of Social Work (Council on Social Work Education).
• One year of social work experience in a healthcare setting.
• Current CPR certification.
• Valid Driver's License, vehicle insurance, and access to dependable transportation.

Additional State-Specific Requirements

• OH: Must be licensed as a Social Worker from the State Board of Social Work.
