Qualifications

• Must hold a current LMSW license in good standing.

• Strong organization, prioritization, and communication abilities.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct the facility's social service programs.
• Develop and implement policies for identifying medically-related social and emotional needs of patients/residents.
• Assist in discharge planning and referrals to community agencies when specialized assistance is required.
