Job summary

The New York State Office of Mental Health at Pilgrim Psychiatric Center is seeking a Licensed Master Social Worker to deliver professional social work services to individuals with mental illness within a multidisciplinary team.

You will conduct culturally competent assessments, provide crisis intervention, develop and monitor treatment plans, and coordinate discharge planning with community resources as part of a recovery‑oriented approach.

Qualifications

• MSW degree from an accredited program.
• Licensed LMSW in NYS.
• Ability to work in a multidisciplinary team.
• Experience with crisis intervention or discharge planning is desirable.

Responsibilities

• Provide crisis intervention and service linkages.
• Develop and monitor treatment plans within the treatment team.
• Determine discharge needs and connect to community resources.
• Establish and maintain relationships with community providers for continuity of care.
