Job summary

AMN Healthcare seeks an LMSW Assessment Specialist to perform psychosocial and risk assessments, determine admission needs, and coordinate discharge planning. You’ll collaborate with an interdisciplinary team and help create aftercare plans to ensure safe transitions for patients.

The role requires an MS in a relevant field, appropriate licensure (LCSW/LMSW/LPC), and BLS/HWS credentials with at least 1 year of acute hospital experience.

Qualifications

• TX LCSW, LMSW, LPC, LPC-A required
• Master's degree in relevant field
• BLS certification required
• HWS certification required
• 1+ year recent acute hospital experience in specialty

Responsibilities

• Conducts comprehensive psychosocial and risk assessments and evaluates patients for admission and level-of-care needs.
• Collaborates with the interdisciplinary treatment team and coordinates crisis intervention services.
• Helps develop discharge and aftercare plans to support safe patient transitions.
