Job summary

Yale New Haven Health offers an in-home care assistant role designed to help Lifetine clients stay safely at home. You will perform personal care, meal planning, light housekeeping, and companionship while following care plans.

Ideal candidates have a High School diploma or equivalent, and CNA/HHA certification is preferred; a valid driver’s license is required to transport clients to appointments. Some experience in home health is highly valued.

Qualifications

• High School diploma or equivalent.
• Minimum 2 years of personal care experience if not CNA/HHA.
• CNA or HHA certification preferred; valid driver's license required.

Responsibilities

• Maintain client confidentiality and privacy.
• Follow each client's care plan and provide outlined services.
• Provide personal care such as bathing, dressing, toileting, grooming.
• Plan and prepare meals according to dietary guidelines.
• Transport client to medical appointments when requested.
• Keep accurate time, attendance, and task records.
• Assist with light housekeeping and laundry.
