Job summary

NorthBay Health seeks an Integrative Primary Care RN to partner with the MD team in an evidence-based lifestyle medicine program. You will guide patients through complex care, coordinate testing and referrals, and deliver high-touch, concierge-like outpatient services.

Responsibilities include comprehensive assessments, patient education, and group classes, with a focus on sustainable behavior change and whole-person care. A valid California RN license is required.

Qualifications

• Graduate from an accredited nursing school; BSN required.
• California state RN license current is required.
• Completion of American College of Lifestyle Medicine course (7.5 hours) within 30 days of start.
• AHA or equivalent BLS required.
• Minimum of two years RN experience.
• Phlebotomy experience required.
• Knowledge of CMS, TJC and EMTALA standards and federal/state regulations.
• Knowledge of telephone-based clinical assessment techniques and NCQA/HEDIS quality standards.

Responsibilities

• Collaborate as part of a 1:1 MD/RN care team within the Integrative Primary Care Program.
• Manage complex care for an outpatient population with chronic health conditions.
• Demonstrate high service orientation in an outpatient program with paid patient access.
• Independently perform RN duties in outpatient settings including comprehensive patient assessment.
• Take vitals, maintain exam rooms, and perform phlebotomy tasks.
• Coordinate referrals, follow up, and schedules testing.
• Document in EHR and other required portals; maintain timely patient communication.
• Collaborate with physicians and the care team to coordinate individualized care plans.
