Job summary

SBNC seeks an experienced Clinic RN in Santa Barbara, CA to provide health advice, education, and care coordination for patients in person, by phone, and via the patient portal. The RN supports the PCMH model and chronic disease management to improve outcomes.

The RN collaborates with primary care teams, maintains confidentiality, coordinates follow-up care after ER visits or hospitalizations, and helps patients activate self-management and health coaching skills.

Qualifications

• Excellent written and verbal communication in English; bilingual Spanish preferred.
• Strong documentation skills and attention to detail.
• Ability to assess patient needs and formulate care plans.
• History-taking and clinical judgment in care coordination.
• Good organizational skills and ability to work under pressure.

Responsibilities

• Provide health advice, education and care coordination for SBNC patients in person, by phone and via patient portal.
• Collaborate with patients’ primary care team to coordinate care.
• Maintain patient confidentiality and HIPAA compliance.
• Coordinate follow-up care after ER visits or hospitalizations.
• Support chronic disease management and patient activation through coaching.
