Job summary

Saint Francis Adult Day Health is seeking a Life Enrichment Program Assistant to lead engaging activities for residents and coordinate plans with families and care teams. You will help create a supportive, person-centered environment in a daytime setting.

Responsibilities include assessing residents for enrichment needs, coordinating activity preferences at admission and during changes, and documenting progress in quarterly notes to enhance quality of life.

Qualifications

• High School Diploma or GED required.
• Experience in healthcare field required.
• Experience with geriatric population preferred.
• Prior Life Enrichment/Activity Aide experience in a skilled nursing community is a plus.

• Facilitate communication between the program and family members.
• Ensure, when applicable, that the Dementia Program is implemented according to the Landmark Lighthouse Program.
• Provides therapeutically friendly environment in the residents’ personal and congregate areas.
• Interviews and assesses all residents for Life Enrichment Program.
• Coordinates completion of preferences for customary routine and activities upon admission, annually or with significant change.
• Coordinates completion of preferences for customary routine and activities for the Care Area Assessments (CAA).
• Assists with the development of person-centered care plans for residents and updates progress in quarterly notes.

Life Enrichment / Activity Aide Program Assistant

Saint Francis Adult Day Health provides a much-needed respite or to enable family members to continue working, provides daytime medical care and supervision for elderly persons and disabled adults over the age of 18 who are unable to remain home unattended.

Some of the job responsibilities of the Life Enrichment Program Assistant / Activity Aide include:

• Implements life enrichment activities on all floors for all residents based on conditions and plans of care.

• Ensure, when applicable, that the Dementia Program is implemented according to the Landmark Lighthouse Program.
