Job summary

Landmark Management Solutions is seeking a Life Enrichment Program Assistant to support residents in a skilled nursing setting. You will implement enrichment activities, coordinate individual preferences, and help maintain person-centered care plans under supervision.

The role emphasizes engagement with residents, families, and care teams, with a focus on geriatric well-being and dementia programming in a supportive environment.

Qualifications

• High School Diploma or GED is required.
• Experience in the healthcare field is required.
• Experience working with geriatric population is preferred.
• Prior Life Enrichment / Activity Aide experience in a skilled nursing community is a plus.

Responsibilities

• Implements life enrichment activities on all floors for all residents based on conditions and plans of care.
• Facilitate communication between the program and family members.
• Ensure, when applicable, that the Dementia Program is implemented according to the Landmark Lighthouse Program.
• Provide a therapeutically friendly environment in residents' personal and congregate areas.
• Interview and assess all residents for Life Enrichment Program.
• Coordinate completion of preferences for routine and activities (MDS Section F) upon admission, annually or with significant change.
• Coordinate completion of preferences for routine and activities for the Care Area Assessments (CAA).
• Assist with development of person-centered care plans and update progress in quarterly notes.

Life Enrichment / Activity Aide Program Assistant

Saint Francis Adult Day Health provides a much-needed respite or to enable family members to continue working, provides daytime medical care and supervision for elderly persons and disabled adults over the age of 18 who are unable to remain home unattended.

Some of the job responsibilities of theLife Enrichment Program Assistant / Activity Aideinclude:

• Implements life enrichment activities on all floors for all residents based on conditions and plans of care.
