Job summary

Pace Organization of Rhode Island is seeking an activities coordinator to support daily Life Enrichment programs at our Day Center. You will assist in implementing exercise groups, devotional activities, arts and crafts, educational programs, manicures, and field trips for participants.

You will travel as needed across Rhode Island, document progress in the electronic health record, and collaborate with nurses, CNAs, and care teams to ensure high-quality activity plans.

Qualifications

Responsibilities

  • Contacts and visits nursing homes regarding Participants that have been placed; assess activity involvement and coordinate with SNF to impact participants.
  • Conducts Initial, Annual, and Semi-annual Life Enrichment assessments per the Outcome Based activity model.
  • Attends all assigned team and care plan meetings.
  • Tracks participant activity attendance and reports to supervisor.
  • Participates in in-service education programs as requested.
  • Assists CNAs with setup and breakdown of mealtimes.
  • Maintains knowledge of special diets and related food selections.
  • Travels throughout the state as needed and requested by DCD.
  • Identifies problems, sets realistic goals, and creates treatment plans for participants in the ADHC and home settings.
  • Documents progress notes and reports changes to the day center nurse.
  • Delivers programming for education, arts and crafts, music, spirituality, etc.
  • Delivers individual and group activities tailored to participants’ needs and limits.
  • Maintains privacy and confidentiality of participants and staff information.
  • Assists in orienting new participants and ensures inclusion into the day center.
  • Stays current on cultural competencies affecting participant care.
  • Documents assessments, orders, and notes in the electronic health record within required timeframes.
  • All other duties as assigned by DCD.

Job Summary

Under the direction of the Day Center Director, assists the Life Enrichment Lead with implementing daily activity programs such as exercise groups, weekly devotions, spelling bees, special event planning, educational programs, manicures, arts and crafts and trips.

Duties/Responsibilities

  • Contacts and visits nursing homes regarding Participants that have been placed. Assesses activity involvement and works with SNF to impact Participants in these settings.
  • Responsible for Initial, Annual, and Semi-annual Life Enrichment assessments in accordance with the Outcome Based activity model.
  • Responsible for attending all assigned team and care plan meetings.
  • Assists in tracking participant’s activity attendance.
  • Attends any appropriate in-service education programs offered in or out of the facility as requested by the DCD.
  • Assists CNAs with setup and breakdown of mealtimes.
  • Knowledge of special diets for participants and maintains appropriate food and snack selections.
  • Required to travel throughout the state when necessary and requested by DCD.
  • Identifies problems, establishes realistic and measurable goals, and sets up treatment plans for participants in the ADHC environment and in the home, setting.
  • Provides documentation into progress notes for significant events related to activities both in the ADHC and in the home setting and reports to the day center nurse any visible changes to the participants.
  • Demonstrates and appreciates programming for regular educational and cultural programs, including exercise, arts and crafts, music appreciation and spirituality, etc.
  • Carries out Individual and group activities tailored to the needs and limitations of participants both in the ADHC and in the home setting.
  • Protects privacy and maintains confidentiality of employees, participants, and agency information.
  • Assists in the orientation of new participants to the program and ensures inclusion into the day center if applicable.
  • Stays current in terms of cultural competencies that impact the care provided to participants.
  • Demonstrates an understanding of the importance of clear, thorough documentation as crucial to good participant care; completes and enters accurate, thorough participant documentation in the electronic health record; enters documentation for assessments, orders, and notes within required timeframes.
  • All other duties as assigned by DCD.

Education Requirements

  • High School Diploma or Equivalent, Required

Physical Requirements

  • Physical ability to regularly stoop, bend, kneel, lift, stand, walk, stretch and reach for extended periods of time.
  • Must be able to lift up to 10 pounds and at times up to 20 pounds.
  • Must have hearing and vision abilities within normal range (corrected) to drive safely and/or to observe and communicate with participants.
  • Must be able to navigate various departments of the organization's physical premises as well as related community care settings.
  • Must be able to tolerate conditions typically associated within a medical office and/or home care setting including potential exposure to bloodborne pathogens and infectious diseases.

Experience

  • 1 Year of Relevant Work Experience, Required
  • 1 Year of Experience working with a frail or elderly population, Preferred
