Job summary

Hilltop Community Resources Inc. is seeking an Activity and Care Specialist in Grand Junction, CO. This part-time role involves providing direct care for adults and supporting daily living activities within the Life Adjustment Program (LAP).

The position requires 1 year of related experience or 2 years of work experience, with a minimum age of 18 (21 for overnight driving). A Colorado driver’s license and CPR/First Aid/QMAP certifications are preferred. Benefits and competitive pay are offered.

Qualifications

Responsibilities

• Demonstrate ethical and professional behavior aligned with Hilltop's values and LAP policies.
• Provide direct, unskilled personal care and hygiene assistance per individualized Care Plans.
• Communicate residents' and staff needs to ensure good customer service and well-being.
• Complete required documentation promptly, accurately, and legibly.
• Provide basic behavior management interventions for brain-injured individuals.
• Maintain professional boundaries with residents, colleagues, and outside agencies.
• Overnight shifts may require driving residents using handicapped-accessible vehicles.
• Assist in housekeeping duties to maintain cleanliness of resident apartments and grounds.
• Attend team meetings and trainings as scheduled.
