YourADH in Brockton, MA seeks an experienced Adult Day Health Social Worker to provide supervision, clinical guidance, and oversight for psychosocial services within the ADH program. You will support the Case Manager with assessments, care planning, referrals, and documentation, while ensuring ethical, culturally responsive care.

Qualifications

• MSW degree preferred and LICSW required in MA.
• LICSW licensed in MA and in good standing.
• Experience in supervision/clinical consultation and case management preferred.

Responsabilités

• Provide regular professional supervision and consultation to the ADH Case Manager, including approximately one hour per week.
• Offer guidance on psychosocial assessments, care plans, goals, interventions, referrals, and follow-up.
• Consult with Case Manager on high-risk or complex client/family situations and suggest interventions.
• Review social work documentation to ensure quality, confidentiality, and regulatory compliance.
• Support coordination of social work services with clients, families, caregivers, providers, and interdisciplinary team members.
• Guide about community resources, benefits, and referrals addressing client needs.
• Support case management and care coordination for older adults, disabled individuals, and other vulnerable populations.
• Provide guidance on MA Adult Day Health requirements, DPH, MassHealth, and related regulations.
• Assist in maintaining proper documentation and audit readiness.
