Job summary

98% Clinical

  • Obtains and documents in electronic medical record (EHR): patient’s vital signs, height, weight, and completes hearing and vision.
  • Interviews patients and documents completely in EHR: history of present illness, medical, family and social history, completes family planning exam components and maternal health follow-up visits, well child checks and treatments, primary care, tuberculosis and STD work-up for medical provider per program protocol.
  • Documents lab tests ordered by medical provider or by program standing order in EHR, labels specimens appropriately, and transports to the lab using appropriate PPE.
  • Administers required screening tests such as ASQ, PHQ, parent pre-visit questionnaire, M-Chat, along with reviewing forms to ensure it is completed by the patient/parent/ guardian.
  • Prepares the client for an exam by the provider, providing appropriate gowns and instructions to the client.
  • Assists medical provider or RN with exams as requested.
  • Maintains adherence to infection control policy by using appropriate PPE.
  • Contacts patients and provides test result information with appropriate documentation in person or by phone as directed by a medical provider or RN.
  • Follow-up on the plan of care developed by a medical provider or RN, discussing treatment options and assisting with scheduling of necessary follow-up appointments.
  • Responds to telephone inquiries by answering questions and makes appropriate referrals to a medical provider or RN.
  • Administers immunizations for children and adults per standing orders and documents in EHR and NCIR.
  • Cleans the room after each use per protocol in preparation for the next patient. Properly cleans and stores equipment per protocol.
  • Stocks rooms with appropriate supplies prior to the start of clinic day, changes needle box when at fill line, and monitors availability of gloves.
  • Follows up on assigned clients' missed appointments, providing a rescheduled visit, and/or documentation in EHR on status.
  • Provides education per plan of care developed by a medical provider or RN.
  • Participates as part of the in-house audit team, viewing medical records as assigned.
  • Assist Communicable Disease/Tuberculous nurse as assigned, in contact investigations.

  • Participates in mandated staff meetings.
  • Attends workshops deemed appropriate by the supervisor.
  • Participates in shelter duty and other activities relating to the Department’s emergency response plan and other disaster relief efforts.
  • Serves as a team member during outreach projects such as flu vaccine season and disease outbreaks.

Performs any other duties and responsibilities as directed.
