Job summary

Elara Caring in Purcell, OK is seeking a Licensed Vocational/Practical Nurse to deliver high-quality home health care, following patient care plans and physician orders while coordinating with the care team.

Ideal candidates hold an LVN/LPN license, nursing degree, and at least 1 year of clinical experience, with the ability to lift up to 50 pounds and reliable transportation; hospice/home health experience is preferred. We offer benefits and opportunities for growth.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.
