VALLEY HOME HEALTH CARE INC in Las Vegas, NV seeks a dedicated LVN to join our home health nursing team. You will provide direct patient care under RN supervision and implement the plan of care in patients' homes.

Responsibilities include wound care, medication administration, chronic disease monitoring, EMR documentation, and patient education. The ideal candidate has Nevada LVN license, 1 year home health experience, valid driver's license, CPR/BLS certification, and the ability to work

Position Summary

We are seeking a dedicated and reliable Licensed Vocational Nurse (LVN) to join our skilled nursing team. As an LVN in home health, you will provide direct patient care under the supervision of a Registered Nurse and in accordance with the plan of care. Visits may include wound care, medication administration, chronic disease monitoring, and patient/family education.

Responsibilities

• Perform skilled nursing visits to patients in their homes in accordance with the care plan
• Accurately document assessments, care provided, and patient response in EMR (Synergy experience preferred)
• Monitor and report changes in patient conditions to supervising RN and physician
• Provide wound care, catheter care, injections, and medication management
• Collaborate with interdisciplinary team to ensure coordinated, patient-centered care
• Educate patients and caregivers to promote independence and safety
• Comply with Medicare/Medicaid and state regulations

Qualifications

• Current LVN license in the State of Nevada (unrestricted)
• One (1) year of clinical experience (home health preferred)
• Valid driver’s license, auto insurance, and reliable transportation
• CPR Certification (BLS)
• Ability to work independently and manage time effectively
• Proficient in electronic documentation (EMR experience preferred)
