Job summary

Elara Caring is seeking a Licensed Vocational / Practical Nurse to join our field team serving the St. Louis and surrounding St. Charles area, including Bridgeton, MO. You will follow individualized care plans and work with the care team to support patients at home.

The role requires an active LPN license, a year of clinical experience, and reliable transportation to travel between patient homes. We offer comprehensive benefits, competitive pay and opportunities for professional growth.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.

Responsibilities

• Follow each patient’s individualized care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

As a Licensed Vocational / Practical Nurse you’llcontribute to our success in the following ways:

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
