Job Summary:

Performs under the supervision of a licensed nurse, carries out the established plan of care for an assigned group of patients. Uses observation, planning, and implementation skills to ensure the performance of direct care activities required to provide and maintain individualized quality patient care. Responsibilities include administration of medications within scope of practice, performing treatments, gathering patient data, and contributing to an individualized care plan under RN supervision. Provides patient education, maintains care records, and ensures hygiene and comfort measures. Upholds standards of AIDET communication and actively contributes to department operations and team communication.

Essential Functions:
  • Perform nursing procedures as regulated by licensure, including medication administration (excluding intravenous and intrathecal), treatments like NG tube passage, enteral feedings, blood glucose monitoring, urinary catheterization, and enemas.
  • Demonstrate competency in data collection, including risk management, education reinforcement, psychosocial and clinical care needs.
  • Coordinate care under RN direction, including patient transfer, discharge, referrals, and psychosocial support.
  • Accurately document patient information such as vital signs, intake/output, height, weight, and observations.
  • Review and consider dietary restrictions, allergies, and preferences to ensure appropriate nutrition.
  • Record glucose monitoring results and QC on EMR, run controls on glucometer and analyzers, and document accordingly.
  • Complete delegated tasks timely and accurately, including unit stock and equipment maintenance.
  • Use clear, professional communication with team members, patients, and families, utilizing AIDET and whiteboards for communication.
  • Collaborate across disciplines for comprehensive patient care, including transfers, discharges, and referrals.
  • Act with urgency when performing tasks and maintain a clean, safe environment.
  • Report equipment or environmental issues for repair and adhere to HIPAA regulations.
  • Utilize SBAR for communication and escalate safety concerns when necessary.
  • Attend monthly training and be flexible to float to other hospital areas as needed.
