Qualifications

Responsibilities

• Observe, assess, document resident condition during rounds and report changes.
• Assure implementation of care plans and nursing interventions as directed.
• Administer medications as prescribed and record administration.
• Conduct rounds with physician and document visits in clinical records.
• Complete narcotic reconciliation and monitor shift changes.
• Oversee aides and ensure proper care delivery.
• Discharge residents per facility policy and complete required forms.
• Communicate with residents’ contacts about status updates or changes.
