Job summary

Crystal Creek Post-Acute in Stockton, CA seeks an LVN to support RNs and lead daily nursing activities in line with facility policies and standards.

Responsibilities include assessing residents, monitoring vitals, administering medications, wound care, and coordinating with CNAs and other staff. A CA LVN license and BLS/CPR are required. We offer comprehensive benefits and a supportive team environment.

Qualifications

• Monitor residents' health status and vital signs.
• Administer medications as prescribed and ensure correct dosage.
• Observe and report significant changes or concerns to RN or physician.
• Provide basic wound care and other treatments as ordered.

Responsibilities

• Conduct initial and ongoing health assessments of residents.
• Monitor vitals, administer medications, observe changes.
• Notify RN/healthcare provider of concerns.
• Administer medications as prescribed.
• Ensure proper dosage and timely administration.
• Monitor and record responses to medications.
• Provide basic wound care as ordered.
• Monitor wound healing and report complications.
• Coordinate with RNs, CNAs, and other staff to plan care.
• Participate in care planning meetings and develop resident care plans.
