Job summary

Sweetwater Care is seeking a Licensed Practical Nurse (LPN) in California to lead nursing activities and ensure high-quality patient care. The role emphasizes infection control, quality improvement, and adherence to physician orders and center policies.

Responsibilities

• Provides nursing care within scope of practice following standards of care and facility policies.
• Implements Infection Control Manual and Quality Improvement Program.
• Utilizes Quality Life Manual for patient placement and monitoring in programs.
• Notifies departments of patient changes (room changes, LOA, admits, discharge).
• Conducts family and patient education.
• Administers medications in a timely manner per facility policy.
• Administers treatments in a timely manner per facility policy.
• Receives, transcribes, and carries out physician orders as permitted by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LPN in state
• One-year prior nursing management experience preferred but not required.
