Job summary
Neighborhood Healthcare, Riverside, CA, seeks an experienced Licensed Vocational Nurse (LVN) to provide direct nursing care within the PACE program. You will monitor participants, coordinate care with the RN/PCP, administer medications, and document in the EHR.
Join an interdisciplinary team focused on promoting health, safety, independence, and quality of life for seniors, with a schedule Monday–Friday, 8am–5pm, and opportunities for ongoing education and impact in the community.
Qualifications
- High school diploma or GED required.
- Valid California LVN license required and maintained.
- Current BLS certification; CPR/First Aid training as required for all PACE employees.
Responsibilities
- Provide direct nursing care to participants in the PACE center, clinic, and/or participant homes as assigned.
- Perform nursing assessments and report changes to RN/PCP.
- Obtain and document vital signs, weights, glucose readings and other data.
- Administer medications and treatments within scope of practice.
- Monitor for medication effectiveness and adverse reactions.
- Educate participants and caregivers on medications and disease management.
- Document nursing care and observations in the EHR.
- Collaborate with the IDT to implement care plans and ensure patient safety.

As a private, non-profit 501(C) (3) community health organization, we serve over 500k medical, dental, and behavioral health visits from more than 100,000 people annually. With two PACE centers located in Riverside County, our PACE program is positioned to serve over 650 senior participants.
The Licensed Vocational Nurse (LVN) provides direct nursing care, participant monitoring, care coordination, and clinical support services under the direction of a Registered Nurse (RN) and/or Primary Care Provider (PCP). The LVN works collaboratively with the interdisciplinary team (IDT) to promote participant health, safety, independence, and quality of life in accordance with PACE regulations and organizational policies.
Schedule: Monday-Friday 8am-5pm.
- Provide direct nursing care to participants in the PACE center, clinic, and/or participant homes as assigned
- Perform nursing assessments within the LVN scope of practice and report changes in participant condition to the RN and PCP
- Obtain and document vital signs, weights, blood glucose readings, and other clinical data
- Administer medications and treatments as prescribed and within scope of practice
- Monitor participants for medication effectiveness, side effects, and adverse reactions
- Assist with wound care and other nursing procedures as delegated
- Ensure infection control and safety standards are maintained
- Collaborate with the interdisciplinary team to implement participant care plans
- Communicate participant concerns, changes in condition, and care needs to the RN and other team members
- Assist with scheduling and coordination of participant appointments, referrals, and follow-up care
- Support transitions of care following hospitalizations, emergency department visits, or skilled nursing facility stays
- Provide health education and reinforce teaching provided by the RN, PCP, and other disciplines
- Educate participants and caregivers regarding medications, treatments, and disease management
- Promote preventive care and healthy lifestyle practices
- Document nursing care, observations, interventions, and participant responses in the electronic health record (EHR)
- Maintain accurate, timely, and complete clinical documentation in accordance with regulatory and organizational requirements
- Report incidents, accidents, and safety concerns per policy
- Comply with all federal and state PACE regulations, California nursing regulations, and organizational policies
- Maintain participant confidentiality and HIPAA compliance
- Participate in quality improvement and performance improvement activities
- Participate in interdisciplinary team meetings as requested
- Assist with participant clinic visits and day center activities as needed
- Respond to participant concerns and emergencies appropriately
- Other duties as assigned

- Highschool Diploma/GED Required
- Valid California Licensed Vocational Nurse (LVN) license required and is to be maintained as a condition of employment
- Current Basic Life Support (BLS) certification through an American Heart Association (AHA)-approved source is required upon hire and must be maintained as a condition of employment
- AHA-approved courses include an in-person, hands-on skills check with a certified instructor using a mannequin to demonstrate CPR and emergency response techniques
- Online-only BLS courses without a live skills check do not meet this requirement.
- In addition, current CPR and First Aid certification are required for all PACE employees in accordance with CMS and DHCS requirements. First Aid training will be provided at onboarding and must be renewed annually
- 1 year of nursing experience preferred
- Experience working with geriatric populations, chronic disease management, home health, long-term care, or PACE programs preferred
- Knowledge of nursing principles and practices within LVN scope of practice.
- Understanding of geriatric care and age-related health conditions.
- Strong assessment, observation, and communication skills.
- Ability to work effectively within an interdisciplinary team.
- Ability to prioritize tasks and manage multiple responsibilities.
- Proficiency in electronic health records and basic computer applications.
- Ability to maintain professional relationships with participants, caregivers, and staff.
- Ability to stand, walk, bend, lift, and assist participants as needed.
- Ability to lift and move up to 50 pounds with assistance when appropriate.
- Ability to travel between participant homes, clinics, and PACE centers as required
- Follows all safety procedures as outlined in Neighborhood Healthcare’s Illness and Injury Prevention Plan (IIPP) and report any injuries and/or unsafe conditions immediately
- Maintains current knowledge of policies and procedures as they relate to safe work practices

- Familiarity with location of nearest fire extinguisher and emergency exits
- Follows all infection control procedures including blood-borne pathogen protocols

Pay range: $35.33 - $50.19 per hour, depending on experience.
The posted salary range reflects the designated pay grade for this position. While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
