Job summary

Neighborhood Healthcare PACE in Riverside, CA is seeking a licensed LVN to provide direct nursing care, participant monitoring, and care coordination under the supervision of a RN/PCP. The LVN will work with the interdisciplinary team to promote health, safety, independence, and quality of life for participants in the PACE program.

Responsibilities

  • Provide direct nursing care to participants in PACE center, clinic, or homes.
  • Perform assessments within LVN scope and report changes to RN/PCP.
  • Record vital signs, weights, glucose, and other data.
  • Administer medications and treatments per order.
  • Monitor medications for effectiveness and adverse effects.
  • Assist with wound care and nursing procedures as delegated.
  • Document care and participant responses in the EHR.
  • Maintain infection control and safety standards.
  • Coordinate with interdisciplinary team on care plans.
  • Support scheduling and referrals for follow-up care.

Neighborhood Healthcare PACE is a managed medical plan built around surrounding participants with a team of physicians, nurses, social workers, therapists and care coordinators to help them maintain good health and a good quality of life. Our goal is to keep our seniors happy and healthy at home surrounded by their family and community.

As a private, non-profit 501(C) (3) community health organization, we serve over 500k medical, dental, and behavioral health visits from more than 100,000 people annually. With two PACE centers located in Riverside County, our PACE program is positioned to serve over 650 senior participants.

The Licensed Vocational Nurse (LVN) provides direct nursing care, participant monitoring, care coordination, and clinical support services under the direction of a Registered Nurse (RN) and/or Primary Care Provider (PCP). The LVN works collaboratively with the interdisciplinary team (IDT) to promote participant health, safety, independence, and quality of lifein accordance withPACE regulations and organizational policies. Schedule: Monday-Friday 8am-5pm.

Responsibilities

  • Provide direct nursing care to participants in the PACE center, clinic, and/or participant homes as assigned
  • Perform nursing assessments within the LVN scope of practice and report changes in participant condition to the RN and PCP
  • Obtain and document vital signs, weights, blood glucose readings, and other clinical data
  • Administer medications and treatments as prescribed and within scope of practice
  • Monitor participants for medication effectiveness, side effects, and adverse reactions
  • Assistwith wound care and other nursing procedures as delegated
  • Ensure infection control and safety standards aremaintained
  • Collaborate with the interdisciplinary team to implement participant care plans
  • Communicate participant concerns, changes in condition, and care needs to the RN and other team members
  • Assistwith scheduling and coordination of participant appointments, referrals, and follow-up care
  • Support transitions of care following hospitalizations, emergency department visits, or skilled nursing facility stays
  • Provide health education and reinforce teaching provided by the RN, PCP, and other disciplines
  • Educate participants and caregiversregardingmedications, treatments, and disease management
  • Promote preventive care and healthy lifestyle practices
  • Document nursing care, observations, interventions, and participant responses in the electronic health record (EHR)
  • Maintainaccurate,timely, and complete clinical documentationin accordance withregulatory and organizational requirements
  • Report incidents, accidents, and safety concerns per policy
  • Comply withall federal and state PACE regulations, California nursing regulations, and organizational policies
  • Maintain participant confidentiality and HIPAA compliance
  • Participate in quality improvement and performance improvement activities
  • Participate in interdisciplinary team meetings as requested
  • Assistwith participant clinic visits and day center activities as needed
  • Respond to participant concerns and emergencies appropriately
  • Other duties as assigned

Qualifications

Education/Experience

  • Valid California Licensed Vocational Nurse (LVN) license requiredandis to bemaintainedas a condition of employment
  • Current Basic Life Support (BLS) certification through an American Heart Association (AHA)-approved source isrequiredupon hire and must bemaintainedas a condition of employment
    • AHA-approved courses include an in-person, hands-on skills check with a certified instructor using a mannequin todemonstrateCPR and emergency response techniques
    • Online-only BLS courses without a live skills check do not meet this requirement.
    • In addition, current CPR and First Aid certification are required for all PACE employees in accordance with CMS and DHCS requirements. First Aid training will be provided at onboarding and must be renewed annually
  • 1 year of nursing experience preferred
  • Experience working with geriatric populations, chronic disease management, home health, long-term care, or PACE programs preferred

Physical Requirements

  • Ability to stand, walk, bend, lift, andassistparticipants as needed.
  • Ability to lift and move up to 50 pounds withassistancewhenappropriate.
  • Ability to travel between participant homes, clinics, and PACE centers asrequired

  • Follows all safety procedures as outlined in Neighborhood Healthcare’s Illness and Injury Prevention Plan (IIPP) and report any injuries and/or unsafe conditions immediately
  • Maintains current knowledge of policies and procedures as they relate to safe work practices

  • Uses appropriate body mechanics to ensure an injury free environment
  • Familiarity with location of nearest fire extinguisher and emergency exits
  • Follows all infection control procedures including blood-borne pathogen protocols
  • Maintains privacy of all patients, employees, and volunteer information and access such information only on a need-to-know basis for business purposes
  • Comply with all regulations regarding corporate integrity and security obligations
  • Reports all behavior and/or activity that are unethical, fraudulent, or unlawful

Pay range: $35.33 - $50.19 per hour, depending on experience.

While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
