POSITION TITLE : Medical Coordinator / LVN

REPORTS TO : Licensed Vocational Nurse Manager

LOCATION : Los Angeles, CA

CLASSIFICATION : Non-Exempt

SALARY RANGE : $28.25 - $31.25 Hourly Rate

PERTINENT INFORMATION :

  • Work Shift: Tuesday - Saturday, 11:30 AM - 8 PM
  • Position involves medical coordination and education.

Medical Coordinator / Licensed Vocational Nurse

The Medical Coordinator works in our Recuperative Care Program to provide post-hospital care, creating a healthy, safe, and home-like environment for guests. Medical Coordinators have excellent bedside manners, are team players, and have a "whatever it takes" attitude. The program provides quality, compassionate care and empowers those we serve.

Responsibilities and Initiatives

To help NHF meet its growth goals, the Medical Coordinator will:

  1. Facilitate hospital discharge processes and conduct patient assessments/engagement for the program.
  2. Perform medical assessment intake and discharge planning.
  3. Maintain current guest charts via the CRM database daily.
  4. Conduct medical/wellness checks, medication education, preventative measures, and follow-up on medical insurance.
  5. Coordinate and provide linkages for guests' medical appointments and follow-up.
  6. Collaborate with Intake to ensure appropriate eligibility for recuperative services and the admission of required DME and Home Health needs.
  7. Participate in weekly case conferences, team meetings, trainings, and in-services.
  8. Provide advocacy for guests as needed, following LVN manager instructions.
  9. Manage a caseload of at least 15 guests, with flexibility to handle 20 or more.
  10. Track and update guest information on the database regarding progress and care plans.
  11. Perform administrative tasks such as charting, guest summaries, and answering phones.
  12. Complete additional duties as required to meet facility needs, without impeding primary responsibilities.
  13. Follow NHF policies and procedures at all times.

Requirements

  • Current LVN License or Bachelor's Degree in Nursing.
  • Current American Heart Association BLS (CPR, First Aid, AED).
  • Recent experience working with homeless patients (preferred).
  • Bilingual in Spanish (preferred).
  • Ability to stay calm and focused under stress.
  • Experience with substance abuse and dually diagnosed guests, and diverse backgrounds.
  • Reliable transportation and current auto insurance if applicable.
  • Effective communication and social skills for building relationships.
  • Ability to meet deadlines, prioritize, and organize effectively.
  • Proficiency in MS Word, Excel, Outlook, web browsing, and typing skills.

Physical Requirements

Frequent standing, walking, extended sitting, bending, stooping, squatting, reaching, twisting, kneeling, and pushing/pulling. Ability to lift up to 50 pounds. Effective verbal communication required. High noise levels in some areas. Must hear alarms, pages, and verbal instructions.
