Position Overview

Title: Medical Coordinator/LVN, Glendale, Ca

Reports To: Licensed Vocational Nurse Manager

Location: Glendale, Ca

Classification: Non-Exempt, Full-Time

Salary Range: $28.25 - $31.25 Hourly Rate

Position Details

This position is to be filled as soon as possible. Work shift: Sunday - Thursday, 9:00AM - 5:30PM.

The Medical Coordinator works in our Recuperative Care Program to provide post-hospital care, creating a healthy, safe, and home-like environment for guests. Responsibilities include patient assessment, medical checks, medication education, and coordinating medical appointments. The role requires excellent bedside manners, teamwork, and a proactive attitude.

Responsibilities
  1. Facilitate hospital discharge and patient assessment/engagement.
  2. Perform medical intake assessments and discharge planning.
  3. Maintain guest charts using CRM database.
  4. Conduct wellness checks, medication education, and follow-up on medical insurance.
  5. Coordinate guest medical appointments and follow-ups.
  6. Participate in case conferences, team meetings, and trainings.
  7. Advocate for guests as needed, following LVN manager instructions.
  8. Manage caseload of 15-20 guests, adjusting as needed.
  9. Track guest progress and care plans in the database.
  10. Perform administrative tasks such as charting and guest summaries.
  11. Complete additional duties to meet facility needs, following policies.
Requirements
  • Current LVN License or Bachelor’s in Nursing.
  • Current AHA BLS certification.
  • Up-to-date TB test and immunizations.
  • Experience with homeless patients (preferred).
  • Bilingual in Spanish (preferred).
  • Ability to work under stress, with diverse populations.
  • Reliable transportation and auto insurance if applicable.
  • Effective communication skills.
  • Proficiency in MS Word, Excel, Outlook, and typing skills.
Physical Requirements

Frequent standing, walking, sitting, bending, lifting up to 50 pounds, and noise level considerations.
