Shift: Saturday and Sunday + 1 weekday, 8:00 am - 8:00 pm

Compensation range: $28 - $32/hour, depending on experience.

Responsibilities:

1. Implement current nursing practice following a comprehensive assessment and the plan of care.
2. Provide accurate and timely documentation of patient services to reflect the plan of care.
3. Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-care plan of care.
4. Participate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary team. Examples include hospice aide, OT, PT, SW, ST, Dietician, or Clinical Supervisor.
5. Use and prepare equipment and supplies effectively, adhering to aseptic technique.
6. Provide appropriate pain and symptom management. Evaluate patient’s response to treatments/medications.

Qualifications:

• Current state license as an LVN
• One (1) year of experience working as an LVN
• Current BLS, hands-on CPR certification
• Valid driver’s license and reliable, insured transportation
