Colonial Care Rehab in Long Beach, CA is seeking a licensed LVN with subacute experience for a full-time, night shift. You will lead CNAs, oversee resident care, administer medications, and complete charting and orders. The role includes admitting, transferring, and discharging residents and coordinating with physicians. PCC knowledge is a plus.

Qualifications

• Active Licensed Vocational Nurse in California.

Responsabilités

• Provide leadership and supervision to certified nursing assistants.
• Oversee the care and wellbeing of residents.
• Prepare and administer medications as ordered by the physician.
• Complete charting and treatment plans/physician orders.
• Admit, transfer, and discharge residents as required.
• Make rounds with physicians as requested/necessary.
• Utilizes the nursing process to assess, plan and implement and evaluate patient care.
• Orient and welcome new residents and families.

Licensed Vocational Nurse (LVN) Requirements

• Active Licensed Vocational Nurse in California


• Fully vaccinated against COVID-19 including Booster shots


• Knowledge of long-term care patient needs


• Demonstrated ability to deal with fast-paced environment

Primary Licensed Vocational Nurse (LVN) Subacute Responsibilities

• Provide leadership and supervision to certified nursing assistants


• Oversee the care and wellbeing of residents


• Prepare and administer medications as ordered by the physician


• Complete charting and treatment plans/physician orders, etc.


• Admit, transfer, and discharge residents as required


• Make rounds with physicians as requested/necessary


• Utilizes the nursing process to assess, plan and implement and evaluate patient care


• Performs interviews to identify specific patient needs


• Assess signs and symptoms indicating changes in patient’s condition


• Collects, analyzes, and interprets information from health care members and documents


• Orient and welcome new residents and families


• Knowledge of Point Click Care (PCC)


• Other duties as assigned

PAY SCALE: $34 to $36

Depending on a number of factors, including, but not limited to location, regularly scheduled work shift, knowledge, skills, experience, and expertise.
