Blue Oak Post Acute is seeking a compassionate Licensed Vocational Nurse (LVN) to join our skilled nursing team. You will provide direct patient care, administer medications, monitor residents, and collaborate with RNs and physicians to ensure high-quality care in a fast-paced environment.

The ideal candidate holds a CA license in good standing, CPR/First Aid certification, and 1+ year of LVN experience. You will observe and document patient status, respond to changes, and support daily nursing

• Reviews reports from previous shifts to determine changes in status and assigns duties
• Turns over accurate records and ensures oncoming shift nurses are updated

Positions Available: Full-time and Per Diem positions available!

Position Summary:

We are seeking a compassionate and dependable LVN to join our skilled nursing team at Blue Oak Post Acute! The LVN will provide direct patient care, administer medications, monitor resident conditions, and collaborate with RNs and physicians to ensure high-quality care for our residents. Responsibilities include documenting patient status, responding to changes in condition, assisting with treatments, and supporting daily nursing operations in a fast-paced STP environment. The ideal candidate is detail-oriented, organized, and committed to delivering excellent patient care.

Qualifications:

• Currently CA license in good standing
• 1+ years' experience preferred
• CPR and First Aid certified
• Positive, professional and calm demeanor in all work-related situations

Responsibilities:

• Observes, reports and documents/charting on patient conditions
• Administers medications in accordance with physician orders
