The LVN (PRN) works closely with CNA’s and CMA’s to ensure timely response to calls for assistance. Oversees resident care including medication administration, obtaining resident vital sign and weight, and reporting any changes in condition to physicians, health care proxies and supervisors.

  • Interviews residents to determine medical problem/condition and documents in chart for physician.
  • Provides services in accordance with state assisted living standards and nurse practice act and maintains compliance to all personnel policies, established community policies and procedures, and Federal/State regulations and standards.
  • Participates in pre-admission and move-in assessments, reviews, and revises assessments with any change in resident’s condition and at frequency determined by the state
  • Completes service plans based on nursing assessments and on change in condition.
  • Assist physician with resident examination, reviewing and transcribing physician orders and provide medical treatment as ordered.
  • Administer medication to residents as ordered by physician.
  • Supervises residents in self-care of medical equipment, such as oxygen, catheters, colostomies, and medication.
  • Maintains supplies to include stocking and monitoring for expiration and documents appropriately for billing purposes.
  • Assists the AL Director with hiring, orientation, training, and scheduling along with supervision and disciplinary process of CNA’s and CMA’s.
  • Provide nursing coverage in memory support.
  • Supervises nursing aides and medication aides during shift.
Educational Requirements and Experience:
  • Active license to practice nursing in the state of Texas.
  • Minimum 1 year of LVN experience.
  • Graduate from an accredited School of Nursing.
