Qualifications

Responsibilities
- Deliver skilled nursing care per each patient’s plan of care
- Monitor patient conditions and communicate changes to the care team
- Administer medications, treatments, and therapies as ordered
- Educate and support patients and family caregivers
- Collaborate with physicians, therapists, supervisors, and care team members
- Maintain accurate and timely clinical documentation
- Advocate for patient health, safety, and well-being
- Provide attentive overnight care and respond to patient needs during your shift
