As a subacute care facility, you'll learn the skills needed to care for ventilator/tracheostomy dependent residents as well as short term rehabilitative patients. New grads welcome! Objective:

The primary purpose of the Licensed Vocational Nurse (LVN) is to coordinate, evaluate, and assist in the delivery of Nursing care and services. You are also responsible for managing the residents' care plans and supervising resident care activities. Both care management and supervisory responsibilities must be executed in accordance with state and federal regulation and facility policies and procedures.

Essential Duties and Responsibilities:

  • Direct day-to-day resident care activities
  • Supervise the Certified Nursing Assistants' delivery of care and proper implementation of the standard of care
  • Complete medical forms, reports, evaluations, studies, charting, etc. as needed
  • Participate in the facility's quality assurance (QA) program as requested or needed
  • Ensure that reference materials (Regulations, Policies and Procedures, etc.) are maintained and organized at the nurses' station
  • Admit, transfer, and discharge residents
  • Review and complete incident/accident reports and conduct investigations as required
  • Interpret facility policies and procedures to staff, residents, visitors, and government agencies as required
  • Attend various facility meetings (care conferences, stand-up meetings, nurses' meetings, Medicare meetings, discharge plan meetings, etc.) as required
  • Receive telephone orders from physicians, process them in accordance with facility procedures, and update the care plan as required
  • Report all care ordered but not delivered, medication errors, charting errors, diet changes, etc. to the Director of Nursing
  • Complete medication and treatment passes as applicable in a timely manner and in accordance with state and federal regulation and facility policy
  • Periodically review medication and treatment administration records for completeness and accuracy and report any discrepancies to Director of Nursing
  • Inform nursing staff of new admissions, including expected time of arrival, room assignment, etc
  • Make daily resident rounds to observe and evaluate the resident's physical and emotional status
  • Meet with residents and/or family as needed
  • Notify attending physician, family, and members of interdisciplinary team when resident is involved in an incident and/or accident or if there is a change in condition
  • Consult with attending physician as needed for treatments, medications, etc
  • Participate in the facility infection control program for monitoring communicable and/or infectious disease among residents and staff
  • Participate in procedures for reporting hazardous conditions or equipment
  • Partner with Director of Nursing to ensure staff coverage requirements are met
  • Give and take report upon beginning and ending of shift
  • Other duties as assigned
