Job summary

Paradigm at Faith Memorial in Pasadena, TX seeks a dedicated Licensed Vocational Nurse (LVN) to join our skilled nursing team. You will provide direct nursing care under supervision, following the plan of care and physician orders, while upholding nursing standards.

We value leadership, organizational skills and a positive attitude; LTC experience is preferred, with six months in rehabilitative/restorative nursing. Knowledge of Point Click Care and MS Office is helpful for daily operations.

Qualifications

• Active LVN license in the state
• 2 years’ Long-Term Care experience preferred
• 6 months of rehabilitative/restorative nursing experience preferred
• Demonstrated leadership and professional attitude required
• Proficiency with Point Click Care preferred
• Computer literacy: Outlook, Office, Internet

Responsibilities

• Provide direct nursing care under supervision of a registered nurse or physician, per nurse practice act and facility policies
• Deliver care to residents following the plan of care and current physician’s orders
• Know and follow facility policies and procedures for day-to-day nursing operations
• Maintain resident health information privacy and report any violations to the Administrator
• Make frequent rounds to ensure nursing staff meet acceptable standards
• Oversee nurse aides and other nursing home staff to maintain department policies and systems

Paradigm at Faith Memorial is a Skilled Nursing Facility that is seeking a dedicated and compassionate LVN to join our team!ResponsibilitiesProvide direct nursing care, under the direct supervision of a registered nurse or physician, in accordance with the nurse practice act, federal and state regulations, facility policies and procedures, and prudent nursing judgment.Deliver care to residents following their plan of care in accordance with current physician’s orders, rules, regulations, and guidelines that govern the nursing care facilities.Have knowledge of and utilize the facility’s written policies and procedures that govern the day-to-day functions of the nursing service department.Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the Administrator.Make frequent rounds of the assigned unit to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards.Oversee nurse aides and communication with and oversight of nursing home staff to ensure that specific policies, procedures, and systems are maintained correctly as it relates to the department.RequirementsMust possess a current, unencumbered, active license to practice as a Licensed Vocational Nurse in the State2 years’ experience in Long-Term Care experience preferred.Six (6) months of experience in rehabilitative and restorative nursing practices preferred.Must be able to demonstrate leadership, organizational skills, and maintain a positive and professional attitudeStrong utilization knowledge of Point Click Care preferredComputer literacy skills (Microsoft Outlook, Microsoft Office, Internet)Paradigm Healthcare was founded on the belief that by empowering each other, we can achieve more and provide better care to every patient we serve.
