Job summary

La Casa Health Care is looking for a qualified Licensed Practical/Vocational Nurse to provide specialized nursing care to patients in Ontario. You will work under the supervision of a Registered Nurse to deliver care, implement nursing plans, and support patients' needs at home.

This position offers flexibility in scheduling with both full-time and part-time options available. With a pay range of $29.00 - $31.00 hourly plus a $500 bonus, this role provides an opportunity to make a significant impact on families.

Qualifications

• Graduate of a state-approved school of practical (vocational) nursing and currently licensed.
• At least 1 year of nursing experience is preferred; new graduates are welcome.
• Current CPR certification required.

Responsibilities

• Administers one-on-one skilled nursing care to patients.
• Implements the nursing care plan for each patient.
• Observes and reports changes in patients' conditions.

Job Summary

A qualified Licensed Practical/Vocational Nurse administers one‑on‑one skilled nursing care to patients on an hourly (shift care) basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with Agency policies.

Qualifications

• Graduate of a state‑approved school of practical (vocational) nursing and currently licensed in the state of California.
• (1) year experience in nursing is a plus, new graduates welcome!
• Current CPR certification.
• Acceptance of philosophy and goals of this Agency.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Understands and adheres to established policies and procedures.
• Implements the nursing care plan for each patient.
• Provides nursing services, treatments, and diagnostic and preventive procedures as assigned.
• Initiates preventive and rehabilitative nursing procedures as appropriate.
• Observe signs and symptoms and report to the physician and RN reactions to treatments, including drugs and changes in the patient’s physical or emotional condition.
• Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
• Prepares clinical and progress notes.
