Job description

Job Summary:

The LVN provides direct nursing care to medically fragile residents in a CLHF setting. Under the supervision of an RN or physician, the LVN ensures high-quality, compassionate care tailored to each patient’s clinical needs.

Key Responsibilities:

Administer prescribed medications and treatments accurately.

Provide skilled care for residents with tracheostomy, ventilator dependence, G-tubes, and wound care needs.

Monitor and record vital signs, intake/output, and patient status changes.

Communicate with an interdisciplinary team regarding resident conditions and care plans.

Document all care provided in accordance with state regulations and facility policies.

Respond promptly to patient needs and emergencies.

Assist with ADLs (activities of daily living) when necessary in coordination with CNAs.

Support infection control and safety protocols.

Requirements:

Current LVN license in the state of California.

Valid BLS/CPR certification.

Previous experience in a subacute, CLHF, or skilled nursing environment preferred.

Familiarity with ventilator and trach care is highly desirable.

Strong documentation and communication skills.

Ability to work independently and collaboratively in a small team setting.

Physical ability to perform nursing duties, including lifting, bending, and long periods of standing.

Shift & Pay:

Shifts: 12-hour shifts, the rate is based on competitive compensation, depending on experience.
