La Casa Health Care in Hemet, CA is seeking a Licensed Vocational Nurse to provide one-on-one skilled nursing care in patients’ homes on an hourly shift-care basis. Morning and night shifts are available; both full-time and part-time roles are open.

You will follow physician orders and the plan of care under the supervision of a Registered Nurse, document progress, teach families, and ensure safety and comfort for clients.

Qualifikationen

• Licensed Vocational Nurse in California (CA) active license.
• 1 year nursing experience is a plus; new grads welcome.
• Current CPR certification.
• Ability to exercise initiative and independent judgment.

• Implement nursing care plan for each patient.
• Provide nursing services, treatments, and related procedures as assigned.
• Initiate preventive and rehabilitative nursing procedures as appropriate.

• Prepare clinical and progress notes.

Job Summary

A qualified Licensed Practical/Vocational Nurse administers one-on-one skilled nursing care to patients on an hourly (shift care) basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with Agency policies.

Qualifications

• Graduate of a state-approved school of practical (vocational) nursing and currently licensed in the state of California.
• (1) year experience in nursing is a plus, new graduates welcome!
• Current CPR certification.
• Acceptance of philosophy and goals of this Agency.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Understands and adheres to established policies and procedures.
• Implements the nursing care plan for each patient.
• Provides nursing services, treatments, and diagnostic and preventive procedures as assigned.
• Initiates preventive and rehabilitative nursing procedures as appropriate.
• Observe signs and symptoms and report to the physician and RN reactions to treatments, including drugs and changes in the patient’s physical or emotional condition.
• Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
• Prepares clinical and progress notes.
