Job summary

Qualifications

• Active, unencumbered LVN license in the practice state.
• Current CPR certification.
• TB skin test completed within last 12 months.
• Twelve (12) months of hands-on nursing experience.
• Reliable transportation.

Responsibilities

• Deliver and coordinate quality patient care in compliance with physician orders.
• Observe, assess, and report changes in patient condition.
• Document activities, assessments, nursing actions, and care coordination promptly.
• Participate in, implement, and update the nursing care plan.
• Take appropriate nursing actions to achieve outcomes.
• Identify changes requiring intervention and modify care to reduce risk.
• Adhere to Aveanna policies, state and federal regulations, and the Nurse Practice Act.
• Provide care using infection control measures per OSHA standards.
• Educate patients and families on disease process, self-care, and prevention.
• Maintain ongoing knowledge through education and skills assessments.

Licensed Vocational Nurse – LVN

Compensation and Schedule

• Base Rates: $27-30/HR
• Case Rates: $27-35/HR
• Up to $35/HR for specific cases
• Full-time night, on-call, or flexible NOC shifts available

Qualifications

• Must have and maintain an active, unencumbered LVN license
• Current CPR certification (hands-on component)
• TB skin test (current within last 12 months)
• Twelve (12) months of hands-on nursing experience
• Reliable transportation

Requirements

• Graduate of an accredited school of nursing
• Current, unrestricted state license as a Licensed Nurse in the practice state
• Current CPR certification
• Demonstrated proficiency in clinical assessments, documentation, and adherence to policies

State-Specific Requirements

• South Carolina – 1 year pediatrics experience
• California – 1 year experience under current nursing license
• Louisiana – 1 year experience as a licensed nurse
• Continuing Education as required by state

Preferred

• Six months of recent experience as a Licensed Nurse in a clinical setting
• Home health experience

Skills and Physical Requirements

• Attention to detail, time management, problem-solving, and communication
• Ability to travel; lift up to 50 pounds; assist with patient repositioning
• English proficiency; ability to read, write, and understand medical documentation

• Ability to function in varied environments with exposure to allergens
