Integrated Real Estate Group in Texas is seeking a licensed LVN to join our community care team on a 6am–2pm shift. The LVN delivers high-quality nursing care under supervision, implements care plans, and supports residents' well-being in a safe living environment.

Ideal candidates have a Texas LVN license, at least one year of nursing experience (geriatrics preferred), and a commitment to continuing education. We offer comprehensive benefits and an equal opportunity workplace.

Qualifications

• Graduate of an accredited LVN or LPN program.
• Current Licensed Vocational Nurse (LVN) license in Texas.
• Minimum of 1 year nursing experience in a healthcare setting; geriatrics preferred.
• Must maintain required continuing education credits.
• Experience or training in Alzheimer's/dementia care preferred.

Responsibilities

• Evaluate residents' health needs and monitor conditions.
• Assist residents with ADLs, personal hygiene, and mobility.
• Administer medications and treatments per orders; monitor for adverse effects.
• Document and maintain accurate resident records.

LVN, 6am - 2pm

About the Role:

The Licensed Vocational Nurse (LVN) delivers high-quality nursing care to residents within the community, ensuring services are provided in accordance with established clinical standards and practices. Working under the supervision of a licensed nurse or healthcare manager, this role is responsible for implementing resident care plans safely and effectively. The LVN supports residents' overall well-being by providing compassionate care, monitoring health conditions, and contributing to a positive, safe, and supportive living environment.

Responsibilities:

• Resident Assessment: Evaluate residents' physical, psychological, environmental, and social needs, identifying potential health concerns.
• Care Delivery: Assist residents with activities of daily living (ADLs), personal hygiene, and mobility while ensuring safe and high-quality care.
• Health Monitoring: Collect and analyze health information, maintaining accurate and up-to-date resident records.
• Treatment Implementation: Administer medications and treatments per physician orders, monitor for adverse reactions, and report concerns promptly.
• Collaboration: Communicate with care teams, physicians, and supervisors regarding changes in condition, hospitalizations, discharges, and appointments.
• Care Planning: Implement individualized service and care plans as directed by the Director of Health Services.
• Infection Control: Follow universal precautions and infection control protocols in all aspects of care delivery.
• Emergency Response: Recognize and respond to emergencies or life-threatening situations, providing first aid and initiating appropriate actions.
• Resident Education: Support residents in developing self-care skills and understanding health practices.
• Team Support: Participate in team meetings, collaborate with nursing staff, and attend in-service training programs.
• Documentation: Maintain accurate, timely documentation in accordance with service and care plan requirements.
• Additional Duties: Perform other assigned tasks while adhering to all company policies and procedures.

Qualifications:

• Education: Graduate of an accredited LVN or LPN program.
• Licensure: Current Licensed Vocational Nurse (LVN) license in the state of Texas.
• Experience: Minimum of one (1) year of nursing experience in a healthcare setting; experience in geriatric nursing, emergency care, and first aid preferred.
• Continuing Education: Must maintain required continuing education credits in accordance with state regulations.
• Specialized Training: Experience or training in Alzheimer's and dementia care, including Validation Therapy, preferred.
