Job summary

Integrated Real Estate Group is seeking a Licensed Vocational Nurse (LVN) to provide high-quality nursing care in our Texas communities. Working under supervision, you will implement resident care plans, monitor health, and support daily living activities for residents in a safe, compassionate environment.

Responsibilities include health assessment, medication administration, coordination with care teams, infection control, and ER readiness.

Qualifications

• Graduate of an accredited LVN or LPN program.
• Current Licensed Vocational Nurse (LVN) license in Texas.
• Minimum 1 year nursing experience; geriatric nursing, emergency care or first aid preferred.
• Must maintain required continuing education credits per state regulations.
• Experience or training in Alzheimer’s and dementia care preferred.

Responsibilities

• Evaluate residents’ physical, psychological, environmental, and social needs.
• Assist with ADLs, personal hygiene, and mobility with safe, high‑quality care.
• Collect and analyze health information; maintain accurate records.
• Administer medications per physician orders and monitor for adverse reactions.
• Communicate changes in condition with care teams and supervisors.
• Implement individualized service plans as directed by Health Services Director.
• Follow infection control protocols in all care activities.
• Respond to emergencies and provide first aid as needed.
• Support residents in self‑care and health education.
• Participate in team meetings and training; maintain proper documentation.
• Adhere to all company policies and perform other duties as assigned.

LVN, 6am-6pm PRN

About the Role

The Licensed Vocational Nurse (LVN) delivers high-quality nursing care to residents within the community, ensuring services are provided in accordance with established clinical standards and practices. Working under the supervision of a licensed nurse or healthcare manager, this role is responsible for implementing resident care plans safely and effectively. The LVN supports residents’ overall well-being by providing compassionate care, monitoring health conditions, and contributing to a positive, safe, and supportive living environment.

Responsibilities

• Resident Assessment: Evaluate residents’ physical, psychological, environmental, and social needs, identifying potential health concerns.
• Care Delivery: Assist residents with activities of daily living (ADLs), personal hygiene, and mobility while ensuring safe and high-quality care.
• Health Monitoring: Collect and analyze health information, maintaining accurate and up-to-date resident records.
• Treatment Implementation: Administer medications and treatments per physician orders, monitor for adverse reactions, and report concerns promptly.
• Collaboration: Communicate with care teams, physicians, and supervisors regarding changes in condition, hospitalizations, discharges, and appointments.
• Care Planning: Implement individualized service and care plans as directed by the Director of Health Services.
• Infection Control: Follow universal precautions and infection control protocols in all aspects of care delivery.
• Emergency Response: Recognize and respond to emergencies or life‑threatening situations, providing first aid and initiating appropriate actions.
• Resident Education: Support residents in developing self‑care skills and understanding health practices.
• Team Support: Participate in team meetings, collaborate with nursing staff, and attend in‑service training programs.
• Documentation: Maintain accurate, timely documentation in accordance with service and care plan requirements.
• Additional Duties: Perform other assigned tasks while adhering to all company policies and procedures.

Qualifications

• Education: Graduate of an accredited LVN or LPN program.
• Licensure: Current Licensed Vocational Nurse (LVN) license in the state of Texas.
• Experience: Minimum of one (1) year of nursing experience in a healthcare setting; experience in geriatric nursing, emergency care, and first aid preferred.
• Continuing Education: Must maintain required continuing education credits in accordance with state regulations.
• Specialized Training: Experience or training in Alzheimer’s and dementia care, including Validation Therapy, preferred.
