Job description

Aarris Healthcare is committed to serving healthcare facilities (assisted living facilities, continuous care, skilled nursing facilities, and rehabilitation centers) throughout California to provide them with qualified and professional medical personnel.

Primary Responsibilities

The LVN will assist in ensuring the provision of quality care that promotes the highest practicable physical, and psychological well-being of each resident as determined by resident assessments and individual plans of care.

Essential Functions

  1. Observe, assess, document, and report resident condition / changes during rounds.
  2. Ensure implementation of care plans as directed.
  3. Administer medications as prescribed.
  4. Conduct rounds with physicians and document visits.
  5. Perform narcotic reconciliation with incoming and outgoing shifts.
  6. Identify significant changes in the condition of residents and take necessary action per facility policies and procedures.
  7. Oversee and monitor CNA staff.
  8. Initiate and complete Incident and Accident Reports per facility policy.
  9. Complete Nursing Assessments for Physician Calls prior to contacting the physician for any change in resident condition.
  10. Communicate with residents’ points of contact regarding status updates or changes in condition.
