Job summary

Sweetwater Care in Santa Rosa, CA is seeking an LVN/LPN to provide nursing care within scope of practice, following physician orders and center policies. You will document patient changes, participate in rounds with physicians, and ensure compliance with state and local guidelines.

Two key requirements are a valid LVN/LPN license and BLS/CPR certification, with long-term care and PointClickCare charting experience encouraged. Experience in nursing management is preferred but not required.

Qualifications

• Must have long-term care experience.
• Experience with PointClickCare charting.
• Current LVN/LPN license in practicing state.
• BLS/CPR certification or willingness to acquire one.
• Nursing management experience preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
