Qualifications

• Long-Term Care experience required.
• PointClickCare charting experience required.
• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire.
• Nursing management experience preferred but not required.

Responsibilities

• Follows established charting procedures and incident reporting.
• Manages narcotic counting, pharmacy orders, and restraint reduction per policy.
• Documents changes in patient conditions (nutrition, infections, skin changes, weight).
• Utilizes Quality Life Manual for patient identification, placement and monitoring.
• Documents room changes, LOA, new admits, or discharges and notifies relevant departments.
• Rounds with physicians when needed.
