Job description

Under the supervision of the Program Head and / or Program Coordinator, is responsible for but not limited to planning, facilitating, implementing, and coordinating mental health / case management services to adult individuals with mental illness and co-occurring disorders.

Essential Functions

  • Provide basic bedside care, taking vital signs such as temperature, blood pressure, pulse, and respiration. Prepare and give injections and enemas, monitor catheters, apply dressings, treat bedsores, and give alcohol rubs and massages.
  • Monitor clients and report adverse reactions for medications or treatments. Collect samples for testing, perform routine laboratory tests, record food and fluid intake and output.
  • Assist with bathing, dressing, and personal hygiene as pertinent to medical condition.
  • Evaluate clients’ needs, develop care plans, and supervise the care provided by nursing aides.
  • Establish and maintain a therapeutic relationship with clients and family members
  • Provide and / or assist with on-call emergency coverage for medical condition.
  • Assist clients, families; and significant others in understanding illness and gaining their acceptance of and participation in the treatment recommendation.
  • Participate, coordinate and implement cooperative arrangements and / or team conferences with various inter-disciplinary team members in the development and implementation of client’s treatment plan and aftercare plan
  • Coordinate and implement group and / or rehabilitation activities related to medical condition
  • Coordinate services with various public and private community health, mental health, and social service agencies in treatment of clients by exploring community resources, providing linkage, and referrals to ensure continuous system of care.
  • Report program activities and development, crises, and any other pertinent information to appropriate chain of command
  • Transport client(s) to access necessary community resources
  • Maintain and submit billing records in a timely manner. Maintain and update client’s files and documentation in accordance with DMH guidelines
  • Perform other tasks and responsibilities as assigned by the Program Coordinator and / or Program Director.

Minimum Qualifications

  • Current and Active license in Nursing
  • Preferred one year of direct service experience in a hospital or mental health setting which has an interdisciplinary team approach
  • Legal status to work in the United States
  • Must have reliable transportation, valid California Driver License, and proof of automobile insurance coverage
  • Knowledge : Basic Computer software programs (Experience on Microsoft Word and Excel, Internet), Knowledge and ability to relate to people with mental illness, Knowledge of DSM-IV and Mental Illness
  • Skills : Good writing and oral (communication) skills are necessary, Strong interpersonal and social skills and Time management skills necessary
  • Abilities : ability to drive, ability to develop and maintain effective working relations with program and with community representatives and contractors, ability to work both independently and as a member of a team
  • Fully COVID-19 Vaccinated with eligible booster(s)

  • Preferred one year of direct service experience in a hospital or mental health setting which has an interdisciplinary team approach.
  • Ability to speak Spanish or one of Asian languages (Vietnamese, or Chinese preferred)
  • Overtime, holiday, or weekend work may be required
