Are you a compassionate and skilled Licensed Vocational Nurse (LVN) with a heart for hospice care? We're seeking dedicated LVNs with hospice experience to join our team on an as-needed basis, providing vital support to patients and their families during their most challenging times.

Job Type : As-Needed / Flexible Hours

Location : Orange County Area

Responsibilities :

• Provide skilled nursing care to patients in their homes or hospice facilities
• Administer medications and treatments as prescribed by the physician
• Monitor and assess patient conditions, including vital signs and pain management
• Collaborate with interdisciplinary team members to develop and implement patient care plans
• Offer emotional support and education to patients and families
• Maintain accurate and up-to-date patient records and documentation

Requirements :

• Current LVN license in California
• Strong communication and interpersonal skills
• Ability to work independently and as part of a collaborative team
• Compassionate and empathetic approach to patient care
• Reliable transportation and willingness to travel to patient homes / facilities
