Job summary

Omega Home Health Care Services Inc. is seeking a Licensed Vocational Nurse (LVN) to provide skilled nursing care for homebound patients in Los Angeles County, Kern County, Santa Maria, Lompoc, San Luis Obispo, and surrounding areas.

The LVN will schedule visits with patients, working at least two days per week with weekend and holiday coverage as needed. Excellent communication with RNs and physicians is essential.

Qualifications

• Valid LVN license required.
• Driver's license required.
• Active BLS/ACLS/PALS certifications.
• Reliable transportation and proof of auto insurance.
• Self-directed and able to work independently.
• Experience with electronic health records.
• At least 1 year of home health experience.

Responsibilities

• Provide skilled nursing care to clients as directed by the RN and physician's plan of care.
• Promote health through teaching and rehabilitative measures under RN supervision.
• Assist clients in learning self-care techniques.
• Coordinate with other health professionals to maintain continuity of care.
• Prepare progress notes and follow care plans as developed by the RN.
• Assist in procedures with established competency.
• Prepare equipment and materials for treatments observing aseptic technique.
• Keep abreast of changes in nursing care and licensed nurse functions.
• Attend client care conferences and communicate with the RN.
• Report client health status to appropriate health professionals.

Essential Job Functions/Responsibilities/Patient Care:

• Provides skilled nursing care to clients as directed by the registered nurse and physician's plan of care in accordance with agency policies.
• Promotes and maintains client health through teaching and appropriate rehabilitative measures under the guidance of the registered nurse who is assigned to the same case. Utilizes agency educational material as appropriate.
• Assists clients in learning appropriate self-care techniques.
• Helps to achieve and maintain continuity of client care by assisting in planning and exchanging information with other health professionals.
• Prepares clinical progress notes and follows client care plans as developed by the registered nurse and according to agency policy.
• Assists the physician and registered nurse in performing specialized procedures for which competency has been established.
• Prepares equipment and materials for treatments observing aseptic technique as required.
• Keeps abreast of changes and trends affecting nursing care and licensed nurse functions.
• Attends client care conferences, communicates regularly with the registered nurse.
• Communicates current health status and needs of clients to appropriate health professionals.

SCHEDULE:

• This position allows flexibility in that you will schedule your visits with your patients!
• Our patients need care 7 days a week, although you have flexibility in your scheduling, we do ask that you are able to work at least two days each week, one weekend a month, and two holidays per year (one winter and one summer)
• If you have additional jobs and your two available days change each week, we can accommodate you!

$45 - $50 per visit, commiserate with experience
Requirements:

• Reliable Transportation
• Proof of auto insurance
• Must be self-directed, able to work independently with little supervision while meeting performance metrics.
• Physical, mental strength and flexible work hours.
• Ability to remain calm under pressure.
• Proficiency with electronic health records and web-based applications
• At least 1 yr of Home Health Experience is required
