Position Overview

This role requires providing direction, leadership, and accountability for Medical Assistants. Under general supervision, performs various direct, technical nursing care functions for patients across different care settings, following nursing care plans and physicians' orders. The Referral Coordinator manages all patient referrals and processes referral authorizations. Must work cooperatively as a team member, demonstrating trust and respect towards colleagues including clinical and non-clinical staff, physicians, and hospital personnel.

Responsibilities

• Identify routine, urgent, or emergency situations; follow procedures for scheduling and obtaining referrals and authorizations.
• Greet patients professionally; demonstrate excellent customer service; document demographic, insurance, and financial information; generate reports from the computer system.
• Assist physicians by preparing patients for examinations or procedures; assess patient conditions; document history, vital signs, and complaints; perform procedures such as EKG, stress tests, casting, suturing, Point of Care Testing, etc.; coordinate care activities including chronic and transitional care management.
• Expedite authorization requests; prepare and verify referral forms; accurately enter data into the system; notify patients and providers of authorization decisions; track services and follow up on referrals; coordinate home health and DME services.
• Administer medications as prescribed; observe and report adverse reactions.
• Dress wounds, apply compresses, and operate medical equipment.
• Collect and perform tests on samples; sterilize equipment; maintain cleanliness of the facility.
• Order supplies; document incidents; dispose of contaminated items properly.
• Lead Medical Assistants to improve patient flow; monitor workflow; manage staffing and tasks as needed.
• Adapt care approaches to meet the developmental and cultural needs of members served.
• Perform additional duties as assigned.
