Job summary

Laredo West Nursing and Rehabilitation Center is seeking a PRN Licensed Vocational Nurse (LVN) to join our nursing team in Laredo, TX. The LVN will participate in rounds, assess residents, document changes, and administer medications as prescribed.

Must hold an active Texas LVN license and have completed an LVN program. Prior experience in a skilled nursing facility is preferred. Responsibilities include care-plan execution, physician communications, and incident reporting.

Qualifications

• Has completed an LVN program at an accredited vocational school or college.
• Must be an active Licensed Vocational Nurse in the state of Texas in good standing.
• Experience working in a skilled nursing facility is preferred.

Responsibilities

• Observe, assess, document, and report resident condition/changes during rounds.
• Ensure implementation of care plans as directed.
• Administer medications as prescribed.
• Conduct rounds with physician and record visits in clinical records.
• Perform narcotic reconciliation between shifts.
• Identify significant changes in resident condition and act per facility policies.
• Oversee and monitor aides.
• Initiate and complete Incident and Accident Reports per facility policy.
• Discharge residents per facility policy.
• Complete Nursing Assessments for Physician Calls prior to calling the physician.
• Communicate with resident's point of contact on status/update or changes in condition.
• Assist with other projects or tasks as assigned by the Director of Nurses or Administrator.

• Other special projects or functions can be assigned at times by the Director of Nurses and/or the Administrator
