Shifts Needed: M-F 7:30-3:30

Essential Functions:

  • Outline and prepare daily routine(s) for clients as written in Individual Program Plan.
  • Assist clients with bathroom needs.
  • Change clients' undergarments and/or clothing if needed.
  • Prepare daily meals and assist with clean-up.
  • Pick-up/drop-off clients in company vans from/to their home/facility (if necessary).

Education and Experience:

  • Direct Support Professional Certificate Year 1 & Year 2 (Preferred or willing to obtain within the first 6 months of hire)
  • Must have First Aid / CPR and CPI (Crisis Prevention Intervention)
  • Must be at least 21 years of age
  • Must have a California Driver’s License
  • Candidate must successfully pass all pre-employment requirements, including but not limited to a background check.

Physical Requirements:

  • Must be able to lift 50lbs.
  • Able to work both indoors and outdoors in all weather conditions.

Job Summary:
The LVN/LPT is responsible for assisting with the coordination of the home so that the residents' health and safety needs are met. The Lead assists to maintain compliance under the California Code of Regulations (CCR), Title 17 and Title 22 as well as the Lanterman Act (Welfare and Institutions Code). The LVN/LPT is expected to ensure residents maximize independence.

Job Responsibilities:

  • Administer medications to individuals and follow the five rights for verification purposes.
  • Attend residents’ doctor appointments and address any concerns and changes with the attending physician.
  • Assist individuals with activities of daily living such as bathing, grooming, cooking, etc.
  • Assess residents’ wounds, help with dressing changes and breathing treatments, as needed.
  • Audit all medications during a new medication cycle and check the five rights for verification.
  • Perform g-tube insertions for feeding to individuals.
  • Perform diabetes insulin medication injections to individuals.
  • Evaluate residents’ level of consciousness and responsiveness post-seizure and/or any traumatic episodes.
  • Redirect in the event of a behavioral episode and continue to support the individual’s needs.
  • Complete and update client notes system daily. Observe and report any unusual behaviors and injuries.
  • Assess and identify signs and symptoms of infections.
  • Perform basic physical reviews such as checking vital signs and head-to-toe assessments.
  • Participate in community outing events with individuals.
  • Perform other related duties and assignments as required.

Requirements:

  • 1 Year Experience supporting adults with developmental disabilities.
  • Licensed Vocational Nurse or Licensed Psychiatric Technician (Required).
  • Must have First Aid / CPR and CPI (Crisis Prevention Intervention).
  • Must be at least 21 years of age.
  • Must have a California Driver’s License.
  • Candidate must successfully pass all pre-employment requirements, including but not limited to a background check.
